Examples of SFF list in a sentence

  • Schedule a final “last chance” onsite survey for those facilities that have been on the SFF list for more than 18 months and have failed to improve.

  • However, the agency has found that a minority of nursing homes have more problems than others (about twice the average number of deficiencies), more serious problems than most other nursing homes (including harm or injury experienced by residents), and a pattern of serious problems that has persisted over a long period (as measured over the 3 years before the date the nursing home was first put on the SFF list).

  • We do not expect programmatic changes to this program for FY 2018 and will continue policies such as the “last chance survey”, and reviewing progress of all other facilities that have been on the SFF list for more than 12 months.

  • The expectation is that QIOs recruit nursing homes for this task using the SFF list and the priorities and process described by the SAs. The SAs are essential to the success of the SFF initiative.
