Access to the Platform. ICE Trade Vault hereby grants Trusted Source a non-exclusive, non-transferable, revocable license to access and use the ICE Trade Vault Platform as such ICE Trade Vault Platform may exist from time to time and to utilize any hardware, software, systems, and/or communications links (collectively, the “System”) furnished by ICE Trade Vault in conjunction with the ICE SDR Service and in accordance with the ICE Trade Vault Terms (as defined below). Trusted Source understands and agrees that Trusted Source shall not be entitled to receive any services other than the ICE SDR Service, express or implied, in the form offered by ICE Trade Vault from time to time pursuant to this ICE Trade Vault Agreement.

Access to the Platform. 5.1 Subject to Client complying with its obligations under this Agreement, Xplor grants to Client a non-exclusive, non- transferable, non-sub-licensable licence for the Licence Period to access the Solution solely for the purpose of enabling Permitted Users to use the Solution Functionality.

In this e-mail or on the Employer Portal, OOPKOP will ask Employer to provide the following information of the Employees: (a) first name and last name; (b) business e-mail address; (c) managers yes or no; (d) language Employees; (e) if applicable: Budget (Development/Well-Being Budget or Personal Budget) per Employee. 5.2 Within 24 hours after the (digital) kick-off, OOPKOP shall send to Employees their personal login details. Subsequently Employees can finalize their account so that they get access to the Platform (and optionally the Marketplace in case Employer added an Additional Package). 5.3 In order to finalize their account, Employees will have to agree to the Terms of Use. 5.4 All accounts are personal. An account cannot be transferred to another Employee. 5.5 Non-completion of an account of an Employee or non-use of an account does not relieve Employer of its obligation to pay Subscription Costs for that Employee.

Access to the Platform. 1. In order to be able to use the Platform an End User or User is required to log in via the Website using the Login Details, after which they will be able to access the Account. Floridata will issue Login Details to the End User for the agreed number of Users. It is not permitted to allow more than one User to use a single set of Login Details. The End User and User are responsible for keeping the Login Details secret. 2. The Login Details issued by Floridata can be changed by an End User and/or User. Any change to Login Details is for the personal risk of the End User and/or User. 3. The End User is entirely responsible for the Login Details issued to it, as well as all actions performed on the Platform by it and/or the Users using those Login Details. Floridata may assume that the User is actually the person who logs in with the Login Details. Floridata is never liable for misuse or loss of the Login Details. 4. As soon as an End User and/or User knows, or has reason to suspect, that a set of Login Details has ended up in the hands of unauthorised persons, they will immediately inform Floridata, without prejudice to their own obligation to take effective measures immediately, such as changing the Login Details. The End User indemnifies Floridata against all third-party claims which are the consequence of, or are connected to, the use of the Login Details issued to the End User. 5. The End User is itself entirely responsible for the purchase and/or correct operation of the (ICT) systems and infrastructure required to use the Service, including the Internet connection. The End User is not entitled to any payment if it is unable to use the Service for whatever reason which falls outside Floridata's responsibility.
Access to the Platform. (a) After Radialogica has taken any steps that it, in its sole discretion, deems appropriate to verify that the organization on behalf of which you have requested access to the Platform is authorized to provide radiation oncology services to individuals treated by a Radialogica Customer or covered by a plan administered by a Radialogica Customer, or otherwise authorized to submit data through the Platform, you will receive an email invitation that will allow you to create or access (as applicable) the Organization Account. (b) Your access to the Organization Account will be through a unique user name and password you create. You may not allow any other person to use your user name and password to access the Platform; you may not use any other person’s user name and password to access the Platform; and you may not otherwise permit the use of the Platform via your user name and password by anyone other than you. (c) You must keep confidential all login credentials (e.g., user name, password, or access code) used to access the Platform or the Organization Account. You are solely responsible for any and all activity that occurs in connection with or via the use of your login credentials, including any losses incurred by you resulting from unauthorized use of such login credentials. You acknowledge and agree that you will provide complete and accurate information in connection with your use of, access to, and requests for access to the Organization Account and that you will promptly update such information as necessary to ensure that it remains complete and accurate. Radialogica reserves the right to terminate your access to the Organization Account and the Platform immediately in the event any such information is found to be inaccurate, incomplete or not current at any time. (d) You and the Organization are responsible, and Radialogica is not responsible: (i) for any internet access, hardware, or software that is necessary or appropriate to facilitate your use of or access to the Platform or Organization Account; (ii) for establishing and following appropriate procedures to safeguard the security of the hardware, software, and information used by you in connection with your access to or use of the Platform or the Organization Account; and (iii) for adequate protection, security and backup of data (including Input Data) and equipment used in connection with your access to or use of the Platform or Organization Account.
