Accident Reporting. 25.1 If You or an Authorised Driver has an Accident or if the Vehicle is stolen You must report the Accident or theft to Us within 24 hours of it occurring and fully complete an Accident/Theft report form.
25.2 If the Vehicle is stolen or if You or an Authorised Driver of the Vehicle has an Accident where:
(a) any person is injured;
(b) the other party has failed to stop or leaves the scene of the Accident without exchanging names and addresses; or
(c) the other party appears to be under the influence of drugs or alcohol, You or the Authorised Driver must also report the theft or Accident to the Police.
25.3 If You or an Authorised Driver has an Accident You and the Authorised Driver must:
(a) exchange names and addresses and telephone numbers with the other driver and drivers licence details;
(b) take the registration numbers of all vehicles involved;
(c) take as many photos as is reasonable showing:
(i) the position of the Vehicles before they are moved for towing or salvage;
(ii) the Damage to the Vehicle;
(iii) the damage to any third party vehicle or property; and
(iv) the general area where the Accident occurred, including any road or traffic signs;
(d) obtain the names, addresses and phone numbers of all witnesses;
(e) not make any admission of fault or promise to pay the other party's claim or release the other party from any liability;
(f) forward all third party correspondence or court documents to Us within 7 days of receipt together with a fully completed Accident Report Form (if not already submitted); and
(g) co-operate with Us in the prosecution of any legal proceedings that We may institute or defence of any legal proceedings which may be instituted against You or Us as a result of an Accident, including:
(i) attending Our lawyer's office; and
(ii) any Court hearing.
Accident Reporting. If a death, serious personal injury, or substantial property damage occurs in connection with the performance of this Agreement, Contractor shall immediately notify the ____________. Contractor shall promptly submit a written report, in such form as may be required by the Court, of all accidents, which occur in connection with this Agreement. The report must include at least the following information: Name and address of the injured or deceased person(s); Name and address of Contractor’s Subcontractor, if any; Name and address of Contractor’s liability insurance carrier; A description of the circumstances surrounding the accident, whether any of the Court’s equipment, materials or staff were involved and the extent of damage to Court and/or other property; and A description of what effect, if any, the accident will have upon Contractor’s ability to perform the Services.
Accident Reporting. The Contractor shall maintain an accurate record of all accidents occurring during the performance of this contract resulting in personal injury, occupational illness or damage to Government property and shall promptly report each accident to the cognizant Government installation Safety Office. A copy of Xxxxxxx'x Compensation reports or information provided telephonically to the Safety Office shall be furnished to the TOCO within seven (7) calendar days. The Contractor shall provide written documentation as required by the installation Occupational Safety and Health Administration Office.
Accident Reporting. If a death, serious personal injury, or substantial property damage occurs in connection with the performance of this Agreement, Contractor shall immediately notify the Supervising Department by telephone. Contractor shall promptly submit a written report, in such form as may be required by Supervising Department, of all accidents which occur in connection with this Agreement. This report must include the following information: (1) name and address of the injured or deceased person(s); (2) name and address of Contractor's subcontractor, if any; (3) name and address of Contractor's liability insurance carrier; and (4) a detailed description of the circumstances surrounding the accident, whether any of the County's equipment, tools, materials or staff were involved and the extent of damage to County and or other property; (5) determination of what effect, if any, accident will have upon Contractor's ability to perform services.
Accident Reporting. Any UAF Local 1324 Unit Member who is injured or who is involved in an accident during the course of his or her employment, no matter how slight the injury, shall file an accident report with the Fire Chief or designee, as soon as possible after the injury or accident and prior to the end of the workday, whenever possible.

(b) If the Vehicle is stolen or if You or an Authorised Driver of the Vehicle has an Accident where:

(iii) the other party appears to be under the influence of drugs or alcohol, You or the Authorised Driver must also report the theft or Accident to the Police.
Accident Reporting. 1. Any duty-related accident which results in any injury or property damage shall be reported to the appropriate immediate supervisor within one (1) hour by any accident-involved employee. If the employee is incapacitated or other extenuating circumstances exist, rendering the employee unable to report the injury or illness within 24 hours, then the employee or their representative shall report the injury or illness as soon as physically possible.
2. Any duty-related accident which does not result in any injury or property damage shall be reported to the appropriate immediate supervisor by any accident-involved employee by the end of the work-day schedule in which the accident occurred.
Accident Reporting. CONTRACTOR shall provide ICTC with immediate telephone 26 notification of accidents. CONTRACTOR shall forward written copies of accident 27 reports within one (1) business day for injury accidents and three (3) business days for 28 non-injury accidents. CONTRACTOR shall also forward all California Highway Patrol 1 (“CHP”) Safety Compliance Reports within two (2) business days after CHP submission 2 to CONTRACTOR.
Accident Reporting. In the event of an accident or injury, the Tournament or League Director is required to fill out an Accident Report Form and submit it to the City of Medford Parks and Recreation Department as soon as possible.
Accident Reporting. JOC promptly shall notify City of any Work site accident involving any Job Order personnel and/or damage to any material and/or equipment. Copies of any injury reports or accident investigation reports generated by JOC or its Sub-Consultants and/or Subcontractors shall be provided to City.
