Common use of Accountings of Disclosures Clause in Contracts

Accountings of Disclosures. Business Associate shall document disclosures of PHI as required to provide Covered Entity with information sufficient to respond to any request by an individual for an accounting of disclosures in compliance with 45 C.F.R. § 164.528, and shall provide such information to Covered Entity upon request within [insert number] business days.87 If Business Associate receives an individual’s request for an accounting of disclosures, Business Associate shall forward that request to Covered Entity within [insert number] business days.
