Additional Payment Provisions. In addition to the terms set forth in Attachment B of the Participating Addendum, the following payment terms shall apply to this Purchase Order: a. Submit invoices to: State of Vermont, [CONTRACTING AGENCY], [CONTRACTING AGENCY ADDRESS]. b. Contractor invoices shall include the Participating Addendum No. and Purchase Order No. which appear at top of this Form. c. Payments shall be made only upon approval and acceptance by the State. d.
