6.1. Every patient who accesses the service will be provided with verbal and written advice and information on their symptoms and details of the discussion with the pharmacist. All patients will be offered a patient information leaflet.
6.2. Any verbal advice should be appropriate for the person in terms of language, ability, and culture, recognising the potential for different cultural meanings associated with the possibility of cancer.
6.3. All patients who consent to being part of the evaluation, will be given the evaluation patient information sheet.

9.4 You undertake to seek independent Advice on a regular basis, especially if your circumstances change or you become aware of any tax or other changes which may affect you or the Trust, tailored to your particular circumstances and if you fail to, or choose not to do so, we have no obligation to advise you further to do so.
9.5 We have no fiduciary or other obligations to you or any other person to ensure that any Trust is, or remains, suitable for your purposes or the purposes of any person in respect of tax or legal matters, even if we are aware of any change in the law or in the circumstances of any person or to consider or to advise you of changes in any law or tax regime or to consider or advise you whether any changes to any law or tax regime will affect you, the Trust or any person.
9.6 Any Advice obtained by us in respect of the Trust at our expense shall belong to us and cannot be relied upon by you or any other person connected with, or advising in respect of, the Trust without our written consent.

2.4 Complaints & corrections Complaints relating to the service provided will be logged and acknowledged. The complaint will be initially dealt with under the ESC Complaints Procedure. If not resolved it will be escalated in accordance with that procedure and those of each Member Authority. Responsibility for the compilation, transmission and accuracy of the data is the remit of HRSS and Councils. Where a correction is required Payroll must be notified as soon as possible. Errors must be notified to Payroll within 1 month of it occurring; thereafter recovery will be the responsibility of the employing Council.
