Agents; Subcontractors. Business Associate agrees to ensure that any agent and/or subcontractor that creates, receives, maintains or transmits Protected Health Information on behalf of Business Associate agrees in writing to restrictions and conditions at least as stringent as those that apply to Business Associate pursuant to this Agreement with respect to such information. Business Associate agrees that, in the event that Business Associate becomes aware of a pattern of activity or practice 0 f an agent and/or subcontractor that constitutes a material breach or violation by the agent and/or subcontractor of any such restrictions or conditions, Business Associate shall take reasonable steps to cure the breach or end the violation, as applicable, and if such steps are unsuccessful, to terminate the contract or arrangement with such agent and/or subcontractor.
Agents; Subcontractors. Business Associate will ensure that any agent, including a subcontractor, to whom Business Associate provides XXXX agrees to implement reasonable and appropriate safeguards to protect the confidentiality, integrity, and availability of such EPHI.
Agents; Subcontractors. Contractor agrees to ensure that its agents, including any subcontractor, to whom it provides PHI received from, or created or received by Contractor on behalf of Covered Entity or RCEB, agrees to the same restrictions and conditions applicable to Contractor with respect to such information.

Business Associate shall require that any agent, including a subcontractor, to whom it provides Protected Health Information received from, or created or received by Business Associate on behalf of Covered Entity, agrees to essentially the same restrictions and conditions that apply through this Agreement to Business Associate with respect to such Protected Health Information.
Agents; Subcontractors. The Parties agree to ensure that any agent or subcontractor, to whom the Party provides the Information, agrees to the same restrictions and conditions that apply through this Agreement, with respect to such Information.
Agents; Subcontractors. The External Institution may not provide any GPC Data to any agent or subcontractor. Any agent or subcontractor, or the employer of such agent or subcontractor if the agent or subcontractor is an individual, must enter into the appropriate agreement with the GPC Parties.

The RHIO shall ensure that its agents, including any subcontractor, to whom the RHIO provides Protected Health Information agree to the restrictions and conditions that apply to the RHIO with respect to such information and implement the safeguards required by Section 2 with respect to electronic Protected Health Information. If the RHIO knows or should know of a material breach or violation by a subcontractor or agent that has created, received, maintained or transmitted such Protected Health Information, the RHIO shall take reasonable steps to cure the breach or end the violation, and if such steps are unsuccessful, it shall terminate the subcontract or relevant agreement, if feasible.

You agree to ensure that any agents, including any subcontractors that have access to UPMC PHI (whether received from UPMC, or created or received by You or Your agents or subcontractors) on behalf of UPMC) agree to in writing to the restrictions and conditions set forth in the T&Cs with respect to such UPMC PHI.
Agents; Subcontractors. Business Associate shall ensure that any agent or subcontractor to whom it provides XXX agrees in writing to substantially the same restrictions and conditions that apply throughout this Agreement to Business Associate.
Agents; Subcontractors. Business Associate shall ensure that any agent to whom it provides PHI, agree in writing to the same restrictions and conditions that apply through this Agreement to Business Associate with respect to such information. In addition, Business Associate shall enter into a business associate agreement with each of its subcontractors which meets the requirements of the Privacy Rule and the Security Rule, including the requirements set forth in 45 CFR § 164.308(b)(2) and 45 CFR § 164.504(e)(1)(ii).
