Common use of Amendments of PHI Clause in Contracts

Amendments of PHI. Business Associate, upon Covered Entity’s request, agrees to make any amendment(s) to PHI in a Designated Record Set that is in the custody or control of Business Associate, to enable the Covered Entity to fulfill its obligations to pursuant to 45 CFR §164.526. If an Individual requests an amendment to the Individual’s PHI directly to Business Associate, Business Associate shall promptly forward such request to Covered Entity.

Business Associate, upon Covered Entity’s request, Associate agrees to make any amendment(s) to PHI in a Designated Record Set that is in the custody or control of Business Associate, to enable the Covered Entity to fulfill its obligations directs or agrees to pursuant to 45 CFR §164.526. If § 164.526 at the request of Covered Entity or an Individual requests an amendment to the Individual’s PHI directly to Business Associate, Business Associate shall promptly forward such request to Covered Entity.

Business Associate, upon Covered Entity’s request, Associate agrees to make any amendment(s) to PHI in a Designated Record Set that is in the custody or control of Business Associate, to enable the Covered Entity directs or agrees to fulfill its obligations to in writing pursuant to 45 CFR §164.526. If § 164.526 at the request of Covered Entity or an Individual requests an amendment to the Individual’s PHI directly to Business Associate, Business Associate shall promptly forward such request to Covered Entityin a time and manner mutually agreedupon by both parties.
