Common use of Amendments to PHI Clause in Contracts

Amendments to PHI. To the extent that Business Associate maintains PHI in a Designated Record Set, Business Associate shall make any amendment(s) to the PHI in such Designated Record Set that Covered Entity directs or agrees to pursuant to 45 C.F.R. § 164.526, at the request of Covered Entity or an Individual, and in the time and manner reasonably specified by Covered Entity.

Amendments to PHI. To the extent that Business Associate maintains PHI in a Designated Record Setcentral database on behalf of Covered Entity, Business Associate shall will make any amendment(s) such amendments to the PHI in such a Designated Record Set that as directed or agreed to by Covered Entity directs or agrees to pursuant to 45 C.F.R. § 164.526, at the request of Covered Entity or an Individual, and in the time and manner reasonably specified by Covered EntityCFR 64.526.

Amendments to PHI. To the extent that Business Associate maintains PHI in a Designated Record Set, Business Associate shall agrees to make any amendment(s) to the PHI in such Designated Record Set that Covered Entity directs or agrees to pursuant to 45 C.F.R. CFR § 164.526, at the request of Covered Entity or an Individual, and in the time and manner reasonably specified by Covered Entity.

Amendments to PHI. To the extent that If Business Associate maintains PHI in a Designated Record SetSet for Covered Entity, Business Associate shall must make any amendment(s) to the PHI in such a Designated Record Set that Covered Entity directs or agrees to pursuant to in accordance with 45 C.F.R. § 164.526, CFR §164.526 at the request of Covered Entity or an Individual, and in the time and manner reasonably specified designated by Covered Entity.

Amendments to PHI. To the extent that Business Associate maintains PHI in a Designated Record Set, Business Associate shall agrees to make any amendment(s) amendments to such PHI at the PHI in such Designated Record Set that direction of Covered Entity directs or agrees to pursuant to 45 C.F.R. § 164.526, at 164.526 within ten business days of receiving the written request of Covered Entity or an Individual, and in the time and manner reasonably specified by Covered Entity, email to suffice.

To the extent that If Business Associate maintains PHI in a Designated Record SetSet for Covered Entity, Business Associate shall must make any amendment(s) to the PHI in such a Designated Record Set that Covered Entity directs or agrees to pursuant to in accordance with 45 C.F.R. § 164.526, CFR §164.526 at the request of Covered Entity or an Individualindividual, and in the time and manner reasonably specified designated by Covered Entity.
