Annual Affidavit. 1. The adoptive parent(s) shall submit the Virginia Annual Affidavit for Adoption Assistance to the local department annually, within thirty 30 days of the anniversary date of the effective date for this executed agreement. The annual affidavit shall be signed by at least one of the adoptive parent(s). • When there is only a one-time payment on the adoption assistance agreement and no other payment or services will be received, an annual affidavit is not completed annually (e.g. non-recurring expenses only). • When there is a $0 payment and Medicaid, an annual affidavit is submitted to the local department.
2. The local department shall notify the adoptive parent(s) in writing two months before the annual affidavit is due.

Annual Affidavit. Consistent with the goals of section 216.0113, F.S., Contractor acknowledges and recognizes that the Department wants to take advantage of any improvements in price/fees over the course of the Contract period. To this end, throughout the term of the Contract Contractor will provide to the Department annually an affidavit from an authorized representative of Contractor attesting that Contractor is in compliance with Section 4.4, Exhibit 6, of the Contract. A sample Pricing Affidavit is included as Exhibit C of this Amendment.

The relative custodian(s) or successor guardian(s) shall submit the Virginia Annual Affidavit for Kinship Guardianship Assistance to the local department annually, no later than the anniversary date that custody was transferred. The annual affidavit shall be signed by one relative custodian or successor guardian when the kinship guardianship assistance agreement is with one relative or guardian; and signed by both relative custodians or successor guardians when the agreement is with two relatives or guardians.
2. The local department shall notify the relative custodian(s) or successor guardian(s) in writing two months before the annual affidavit is due.
