Attending Physician Statement. CAMH shall provide reimbursement for the cost of the Attending Physician Statement(s) (APS) as it may require from time-to-time to certify an employee’s illness or ability to return to work. The cost of Attending Physician Statement will be reimbursed by CAMH upon provision of a receipt, subject to reasonableness of such receipt.
