Authorizing Prescriber Sample Clauses

Authorizing Prescriber. Authorizing Prescriber hereby represents that neither Authorizing Prescriber, the company under which Authorizing Prescriber submits claims to Medicare (“Group Practice”) nor, to the best Authorizing Prescriber’s knowledge, Group Practice’s employees, agents or independent contractors involved in the provision of services have been excluded from participation in any Federally-funded health care programs, including, but not limited to, Medicare and Medicaid.
