Common use of Beneficiary Informational Materials Clause in Contracts

Beneficiary Informational Materials. Contractor shall provide and make available at initial contact, and shall notify beneficiaries of their right to request and obtain the following information upon request: DMC-ODS Beneficiary Booklet, Provider Directory, DMC-ODS Formulary, Advance Health Care Directive Form (required for adult clients only), Notice of Language Assistance Services available upon request at no cost to the client, Language Taglines, Grievance/Appeal Process and Form, Notice of Privacy Practices, and EPSDT poster (if serving clients under the age of 21). Contractor shall also post notices explaining grievance, appeal and expedited appeal processes in all program sites, as well as make available forms and self- addressed envelopes to file grievances, appeals and expedited appeals without having to make a verbal or written request to anyone. The County will produce required beneficiary informational materials in English and Spanish, and in alternative formats as requested. Contractor shall request materials from the County, as needed.
