Breast Reconstructive Surgery Services. 1. If You receive Benefits in connection with a mastectomy and You choose to have breast reconstruction, Your Plan will pay for the following Covered Services: a. Reconstruction of the breast on which the mastectomy has been performed; b. Surgery and reconstruction of the other breast to produce a symmetrical appearance; and c. Prostheses and physical complications of all stages of mastectomy, including lymphedemas. 2. You will consult with Your attending Physician about receiving these Covered Services.
