Breast Reconstructive Surgery Services. 1. If You are receiving Benefits in connection with a mastectomy and elect breast reconstruction in connection with such mastectomy, You will also receive Benefits for the following Covered Services: a. reconstruction of the breast on which the mastectomy has been performed; b. Surgery and reconstruction of the other breast to produce a symmetrical appearance; and c. prostheses and physical complications of all stages of mastectomy, including lymphedemas. 2. These Covered Services shall be delivered in a manner determined in consultation with You and Your attending Physician, if applicable, and will be subject to any Deductible Amounts, Copayment and Coinsurance.
