Bridge Inspection Follow-Up Action Worksheet (Off. System) — Engineer shall use this form to summarize areas of bridge deterioration and make a recommendation of how the bridge can be repaired. Engineer shall classify and group recommended repairs as follows: Critical – Actions required within 30 days, Urgent – Actions required within 6 months, or Routine – Actions recommended within 24 months. Engineer shall group recommended follow-up actions from previous inspections that have not been performed and are still warranted for the structure together, list again, and note that they were not performed since the last inspection. If immediate action is required to maintain the safety of a structure, Engineer shall notify the TxDOT District Bridge Inspection Coordinator immediately by telephone. Engineer’s inspection team leader shall be available to State for coordination of emergency efforts. If the TxDOT District Bridge Inspection Coordinator is not available, refer to the emergency contact list provided by the TxDOT District Bridge Inspection Coordinator and contact one of the individuals identified on this list. Note: One extra copy of this form is required for each off-system bridge, grouped by owning jurisdiction. Engineer shall submit these copies to the TxDOT District Bridge Inspection Office at the end of the work authorization. For any bridge not requiring maintenance, Engineer shall still submit this form with the first line in the comments section populated with “No action required.”
