BUSINESS ASSOCIATE EXHIBIT. This Business Associate Exhibit (this “Exhibit”) supplements and is made part of the Management Services Agreement to which it is attached as entered into between the Company and the Manager. The Company is a “Covered Entity” and Manager is a “Business Associate” as those terms are defined under HIPAA. The capitalized terms used in this Exhibit that are not otherwise specifically defined have the meanings ascribed to them in the Agreement. Other capitalized terms used in this Exhibit and not defined in the Agreement have the meanings ascribed to them under HIPAA.
BUSINESS ASSOCIATE EXHIBIT. 1. Catch-all definition The following terms used in this Exhibit shall have the same meaning as those terms in the HIPAA Rules: Breach, Data Aggregation, Designated Record Set, Disclosure, Health Care Operations, Individual, Minimum Necessary, Notice of Privacy Practices, Protected Health Information, Required By Law, Secretary, Security Incident, Subcontractor, Unsecured Protected Health Information, and Use.
BUSINESS ASSOCIATE EXHIBIT. If Customer concludes that the Service will include access to Customer Data that is protected by the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), and Customer is a Covered Entity as defined under HIPAA, the parties agree to attach Workday’s Business Associate Exhibit to this Agreement, which shall apply to Workday’s receipt, maintenance or transmission of Protected Health Information from, or on behalf of Customer, as described in such Exhibit.
BUSINESS ASSOCIATE EXHIBIT. This Business Associate Exhibit (“Exhibit”) supplements and is made a part of the Management Services Agreement (the “Agreement”), by and between: 21st Century Oncology of California, a Medical Corporation, and Xxxxxxx Radiation Oncologists, P.C., each of which hereby acknowledges that it is a “Covered Entity” [defined in paragraph l(b) below] and shall hereinafter be referred to as such, and 21st Century Oncology-CHW, LLC, which hereby acknowledges that it is a “Business Associate” [defined in paragraph 1 (a)] and shall hereinafter be referred to as such.
BUSINESS ASSOCIATE EXHIBIT. To address the requirements under the Health Insurance Portability and Accountability Act of 1996, the parties agree to the Business Associate Terms contained in this Agreement.
