CAMBIO DE PLAN. A través del Empleador, en la fecha del aniver- sario, el Titular del Certificado puede solicitar un cambio de plan o de deducible. Algunas solicitudes están sujetas a evaluación de riesgo. Los beneficios pagaderos por cualquier enfermedad o lesión no causada por un accidente o enfermedad de ori- gen infeccioso, estarán limitados ▇▇ ▇▇▇▇▇ de los beneficios provistos por el nuevo plan o por el plan ante- rior. Durante los primeros doce (12) meses después de la fecha efectiva del cambio, los beneficios de mater- nidad, del recién nacido, desórdenes congénitos y trasplante de órganos estarán limitados ▇▇ ▇▇▇▇▇ beneficio provisto por el nuevo plan o por el plan anterior. ACCIDENTE: Evento desafortunado que ocurre de forma imprevista y súbita, provocado por una causa externa, siempre sin la intención de causarlo por parte del asegurado, el cual provoca lesiones o trauma y requiere atención médica ambula- ▇▇▇▇▇ u hospitalización del paciente. La información médica relacionada con el accidente será evaluada por la aseguradora, y la compensación será determinada de acuerdo a los términos y condiciones de la póliza.

  • Option Plan This Option is subject to certain additional terms and ----------- conditions set forth in the Plan pursuant to which this Option has been issued. Optionee acknowledges receipt of a copy of the Plan on file with the Secretary of the Company and, by acceptance hereof, agrees to and accepts this Option subject to the terms of the Plan.   • Limitation on Out-of-State Litigation - Texas Business and Commerce Code § 272 This is a requirement of the TIPS Contract and is non-negotiable. Texas Business and Commerce Code § 272 prohibits a construction contract, or an agreement collateral to or affecting the construction contract, from containing a provision making the contract or agreement, or any conflict arising under the contract or agreement, subject to another state’s law, litigation in the courts of another state, or arbitration in another state. If included in Texas construction contracts, such provisions are voidable by a party obligated by the contract or agreement to perform the work. By submission of this proposal, Vendor acknowledges this law and if Vendor enters into a construction contract with a Texas TIPS Member under this procurement, Vendor certifies compliance.
