If the Subscriber is making any contribution towards coverage for himself or herself, or for Dependents, the Subscriber may request termination of this cov- erage. If coverage is terminated at the Subscriber’s request, coverage will end at 11:59 p.m. Pacific Time on the last date for which Premiums have been paid. Blue Shield may cancel the Subscriber and any De- pendent’s coverage for cause for the following con- duct; cancellation is effective immediately upon giv- ing written notice to the Subscriber and Employer:
