Common use of Care Plan Registry Clause in Contracts

Care Plan Registry. The P3N will host a statewide Care Plan Registry and repository that will allow Medicaid Managed Care Organizations (MCOs), Health Care Providers, and PA eHealth to register patient Care Plan documents that are discoverable and retrievable by querying the P3N. DHS requires physical health and behavioral health MCOs to develop and share care plans for complex patients.
