Case Plan. A court ordered, written document that, at a minimum, specifies the type of home in which a Xxxxxx Child/Non-Minor Dependent (NMD) shall be placed, the safety of that home, and the appropriateness of that home to meet the Xxxxxx Child’s/NMD’s needs.
Case Plan. A court ordered, written document that, at a minimum, 16 specifies the type of home in which a Xxxxxx Youth/NMD shall be placed, the 17 safety of that home, and the appropriateness of that home to meet the Xxxxxx 18 Youth's/NMD's needs.
Case Plan. Upon receipt of the referral, Contractor and client shall jointly develop a case plan to keep track of the client’s long-term goals and milestones. The plan shall include linkages to mental health and/or substance use treatment services, housing resources, legal services, employment services, education, transportation, support groups, and/or other services based on client’s short-term and long-term needs. The case plan should be documented and kept for records.
Case Plan. Case plan is typically reviewed at Case Review Hearing. However, on its own motion or motion of any party for good cause shown, court may consider content/implementation of case plan or any response filed concerning it.
Case Plan. DCFS shall file at least 10 days before hearing; upon filing provide copies to counsel by mail/email and unrepresented parties by certified mail/email per Article 674. Any party may file written response.
Case Plan. Case Plan. A written document which identifies a case plan goal, objectives to be achieved, services to be provided, and case management activities to be performed, and, at a minimum, specifies the type of home in which a Xxxxxx Youth/NMD shall be placed.
Case Plan. A Case Plan shall be completed initially within thirty (30) calendar days from CTS. Any updates to the Case Plan shall be completed as needed throughout the duration of services by Provider. Ongoing contacts with the family and their support system shall occur throughout assessments to determine the appropriate family needs for the Case Plan. The Case Plan shall be entered into FSFN.
Case Plan. Ensure the applicants understand that they will have identified roles and responsibilities for implementing aspects of a xxxxxx child‟s case plan. If there are special considerations to elaborate to determine what resources are needed.
Case Plan. It should include four components:
i) a plan worked out between the caseworker and the PSA/ family members/ significant others with agreed direction on a feasible drug treatment and rehabilitation plan for the PSA;
ii) a specific time frame;
iii) specific actions to be taken by the caseworker and/or the PSA/ family members/ significant others in working towards the agreed direction; and
iv) goals that can be evaluated.
