Chronic Homelessness. Upon determination that the individual has a history of chronic homelessness, Contractor shall complete the Vulnerability Index – Service Prioritization Decision Assistance Tool (VI-SPDAT) in the County’s Coordinated Entry System. Referral policies or procedures are subject to change and subject to the review and approval of both the County and the Contractor.
