Chronically Homeless Clause Samples
Chronically Homeless. Enrollees who meet the definition of “Chronically Homeless" as set forth by the U.S. Department of Housing and Urban Development, described as an unaccompanied homeless individual with a disabling condition who either has been continuously homeless for a year or more, or has had at least four (4) episodes of homelessness in the past three (3) years, as determined by EOHHS.
Chronically Homeless. Refers to a homeless individual who has experienced homelessness for a year or longer, or who has experienced at least four episodes of homelessness in the last three years and has a disability.
Chronically Homeless. Enter the number of clients who have been continuously homeless for a year or more; OR have had a least four episodes of homelessness in the past three years; AND have one of the following disabling, diagnosable conditions: substance use disorder, serious mental illness, chronic physical illness or a disability.
Chronically Homeless. An individual who:
Chronically Homeless. (1) An individual who:
i. Is homeless and lives in a place not meant for human habitation (e.g., living on the streets), a safe haven, or in an emergency shelter; and
ii. Has been homeless and living or residing in a place not meant for human habitation, a safe haven, or in an emergency shelter continuously for at least 1 year or on at least four separate occasions in the last 3 years; and
iii. Can be diagnosed with one or more of the following conditions: substance abuse disorder, serious mental illness, developmental disability (as defined in Section 102 of the Developmental Disabilities Assistance ▇▇▇▇ of Rights Act of 2000 (42 U.S.C. 15002)), post-traumatic stress disorder, cognitive impairments resulting from brain injury, or chronic physical illness or disability;
(2) An individual who has been residing in an institutional care facility, including jail, substance abuse or mental health treatment facility, hospital, or other similar facility, for fewer than 90 days and met all of the criteria in paragraph (1) of this definition, before entering that facility; or
(3) A family with an adult head of household (or if there is no adult in the family, a minor head of household) who meets all of the criteria in paragraph
(1) of this definition, including a family whose composition has fluctuated while the head of household has been homeless.
Chronically Homeless. Homeless Categories
Chronically Homeless. Chronically Homeless as defined for the purposes of Health Through Housing is a homeless adult with a disabling condition who has either been continuously homeless for a year or more or has had at least four episodes of homelessness in the past three years with each episode separated by seven days, and cumulatively totaling twelve months.
