Closeout Reconciliation. Grantee must complete and submit a closeout packet and requested documentation in accordance with the instructions and on the date established by HHSC. Grantee must complete and submit all requests for funds on the forms provided by HHSC and by the date designated by HHSC. A final program report shall be submitted to HHSC on or before the date designated by HHSC, with no fewer than 45 business days advance notice to Grantee. The total of all program reports, including the final program report, must be reconciled to all funds received during the Contract period. Grantees shall submit funding requests to HHSC no later than November 30th for the previous fiscal year, or after the final program report is submitted, unless indicated otherwise by a funding source.
