The driver uses any controlled substance, unless the use is pursuant to a physician’s written certification stating that the use does not adversely affect the driver’s ability to safely operate a motor vehicle. 2. A supervisor or administrative employee has actual knowledge that a driver has used a controlled substance. 3. The driver has a positive confirmed test for a controlled substance. 4. The driver displays behavior or appearance characteristic of controlled-substance use.
