Custodial Services. Contractor shall: A. Thoroughly complete the Custodial Services in accordance with the Contract, in a professional manner, using quality equipment and materials that conform to all current Federal, State and local regulations. B. Oversee that the Custodial Services are completed in accordance with the Contract. C. Ensure the Custodial Services conform with the “Tasks Procedure” section of this Exhibit A to ensure the Facility is uniformly clean, hygienic. D. Provide all labor, materials and equipment necessary for cleaning and maintenance, unless otherwise specified herein. E. Have a supervisor monitor its employees’ arrival and departure or utilize a remote timekeeping system for monitoring employee real-time arrival and departure from the Facility. F. Provide backup staff in the event the primary cleaner(s) or the supervisor is unable to Perform the Custodial Services. Backup staff shall arrive no later than one (1) hour after the scheduled start time. G. Perform all weekly, quarterly, bi-annual and annual Services on Mondays between the hours of 6:00am through 2:30pm, unless otherwise requested in writing by the Client Agency. The work schedule is attached as Attachment 1 to this Exhibit A, Task Schedule Checklist. This schedule is based on a twelve (12) month period, identifying and delineating the time frames for the required Custodial Services. H. Not allow its employees to enter the Facility any sooner than fifteen (15) minutes prior to the start of their shift, nor re-enter following shift end. Contractor and its employees shall sign in and out of the Facility on an approved time sheet provided by the Client Agency Designee. I. Unless otherwise directed, not provide any Custodial Services on all State observed holidays. Additionally, if the State decides to close the Facility on any other day including, but not limited to, employee furloughs, Custodial Services will not be required and should not be provided on the closure date, unless otherwise directed. The Client Agency Designee shall provide Contractor with reasonable advance notice of Facility closures whenever possible. The Contractor shall prorate any invoice relating to a scheduled workday in which Custodial Services were not provided due to Facility closure. The prorated formula will be calculated by the monthly cost divided by the number of Service days within the particular month to get a daily rate; multiply the daily rate by how many days of Service completed within the particular month to establish the prorated monthly cost. The Contractor shall reschedule any task that was not completed due to the Facility closure with the exception of daily tasks. The Client Agency Designee must be notified and approve all task schedule changes in writing. J. Implement a quality control plan to monitor Performance of the Custodial Services; as part of the quality control, the Contractor shall include the following: 1. An inspection plan by the Contractor to monitor that the Custodial Services are in compliance with the standards indicated in the “Task Procedure” section of this Exhibit A. 2. Track quality control utilizing Attachment 1 Task Schedule Checklist. Cleaner(s) shall use the right column(s); initial each weekly task as it is completed. Each week the cleaner(s) shall enter the date on the Task Schedule Checklist and its Supervisor shall list the cleaner(s) name(s) and shall sign the last section as affirmation the Custodial Services were completed and inspected by the Supervisor. The Contractor shall make and have available blank copies of Attachment 1 Task Schedule Checklist each week for the cleaner(s) and Supervisor to use. The Contractor shall provide the Client Agency Designee the completed Attachment 1 Task Schedule Checklist upon its completion each week. 3. The Contractor and the Client Agency shall schedule a date and time to be mutually agreed upon but on at least a bi-annual basis to inspect the Custodial Services utilizing Attachment 2 Inspection Form and applying scores to the Custodial Services. Prior to each scheduled meeting, the supervisor shall document all performance entries from the last month from the Daily Log and all Additional Work completed from the last month to the applicable sections of the Inspection Form. The Contractor shall provide the Client Agency Designee a copy of the completed Inspection Form immediately upon completion. 4. The Inspection Form scoring shall be “A” as acceptable, “NI” as needs improvement, and “U” as unacceptable. After review, the Client Agency shall indicate either an overall acceptance of the Custodial Services or non-acceptance which shall include an explanation and identification of any corrective action that needs to be taken and a timeline for the rework at no additional charge. Completed Inspection Form identifying a task receiving a scores of U shall immediately issue a credit in accordance with the Performance, Non- Compliance section of this Exhibit A. 5. Monitor equipment on Site daily to confirm the safety and integrity of the equipment. Contractor shall immediately remove and replace any unsafe equipment from the Facility. 6. Designate or assign a representative(s) to act on behalf of the Contractor regarding all matters affecting the Custodial Services. The Contractor shall identify this individual upon commencement of the Contract and will use reasonable efforts to ensure the assigned representative remains assigned to this account through the Term of the Contract.

Custodial Services. Contractor shall: A. Thoroughly complete Lessor will provide Venue in neat, presentable fashion at the Custodial Services beginning of the Rental Date(s). Lessor shall also be responsible for the cost of basic janitorial maintenance to the Venue during the Rental Date(s), including dressing areas, public restrooms, and Lobby. Lessee agrees to place all garbage in accordance with an appropriate trash container. Any excess debris should be placed in Lessor’s dumpster at the Contract, in a professional manner, using quality equipment and materials end of each day. Lessee may request that conform to all current Federal, State and local regulations. B. Oversee that Lessor provide additional janitorial services during the Custodial Services are completed in accordance with the Contract. C. Ensure the Custodial Services conform with the “Tasks Procedure” section term of this Exhibit A Agreement for an additional charge. Specific use of areas may incur an additional custodial or dumpster charge. Lessee shall be responsible for maintaining and returning said Venue to ensure Lessor in the Facility is uniformly clean, hygienic. D. Provide same conditions as at the commencement of this Agreement. Storage: Lessor at no time guarantees the use of storage space. All of Lessee’s property must be removed from the Venue upon expiration of the Rental Date(s) unless otherwise agreed to by Lessor. Lessee shall restore and properly store all labor, materials and equipment necessary for cleaning and maintenanceof Lessor used by Lessee, unless otherwise specified herein. E. Have a supervisor monitor its employees’ arrival prior arrangements have been made and departure or utilize a remote timekeeping system for monitoring employee real-time arrival agreed to by Lessor. Lessor reserves the right to remove all property of Lessee remaining after the expiration of the Agreement at the expense of Lessee and departure from to store the same at the expense of Lessee without any liability on the part of Lessor. Parking: Parking is available along the street and in surface lots around the Facility. F. Provide backup staff . Lessor is not responsible for providing parking for Lessee’s event and cannot guarantee that other events in the event downtown area will not affect the primary cleaner(savailable parking for Lessee’s customers, guests, and patrons. Parking is NOT guaranteed under this Agreement. Time and Intermission: Should the Event necessitate a Box Office; it will open sixty (60) minutes prior to each performance. Additionally, the Venue must be accessible to attendees a minimum of thirty (30) minutes prior to each performance for seating. If the program is one hundred (100) minutes or the supervisor is unable longer (total time), there shall be an intermission of between fifteen (15) and twenty (20) minutes in length. Exceptions to Perform the Custodial Services. Backup staff shall arrive no later than one (1) hour after the scheduled start time. G. Track quality control utilizing Attachment 1 Task Schedule Checklist. Cleaner(s) shall use the right column(s); initial each weekly task as it is completed. Each week the cleaner(s) shall enter the date on the Task Schedule Checklist and its Supervisor shall list the cleaner(s) name(s) and shall sign the last section as affirmation the Custodial Services were completed and inspected by the Supervisor. The Contractor shall make and have available blank copies of Attachment 1 Task Schedule Checklist each week for the cleaner(s) and Supervisor to use. The Contractor shall provide the Client Agency Designee a copy of the completed Inspection Form immediately upon completion. 4. The Inspection Form scoring shall be is as follows: “A” as acceptable, “NI” as needs improvement, and “U” as unacceptable. After review, the Client Agency shall indicate either an overall acceptance of the Custodial Services or non-acceptance which shall include an explanation and identification of any corrective action that needs to be taken and a timeline for the rework at no additional charge. Completed Inspection Form identifying a task receiving a scores of U Contractor shall immediately issue a credit in accordance with the Performance, Non- Non-Compliance section of this Exhibit A.A 5. Monitor equipment on Site daily to confirm the safety and integrity of the equipment. Contractor shall immediately remove and replace any unsafe equipment from the Facility. 6. Designate or assign a representative(s) to act on behalf of the Contractor regarding all matters affecting the Custodial Services. The Contractor shall identify this individual upon commencement of the Contract and will use reasonable efforts to ensure the assigned representative remains assigned to this account through the Term of the Contract.
