Common use of Data Certifications Clause in Contracts

Data Certifications. All data, reports, documents, records, encounter data, and any other information required to be submitted to the Division by the Contractor shall be certified by one of the following: the Contractor’s Chief Executive Officer, the Contractor’s Chief Financial Officer, or an individual who has delegated authority to sign for, and who reports to, the Contractor’s Chief Executive Officer or Chief Financial Officer. The certification must attest, based on best knowledge, information, and belief, as follows: To the accuracy, completeness and truthfulness of the data; to the accuracy completeness and truthfulness of the documents. The Contractor must submit the certification at the time the certified data, documents, reports, records, encounter data, or other information is submitted to the Division.
