1. All forms must be accurate and complete in every detail and must be signed by an officer of Developer. Percentages and dollar amounts must be accurate, listed to two decimal places and not rounded up or down.
2. A separate DBE Intended Participation Affidavit must be submitted for each DBE used to meet the DBE Goals. Developer shall indicate each DBE's name, address, a description of the work the DBE will perform, proposed Subcontract amount and the NAICS code applicable to the kind of work the firm would perform on the Project.
