Dermatological Services. AvMed will cover office visits to a dermatologist for Medically Necessary Covered Services, subject to the Limitations described in Part X.
Dermatological Services. Prior Authorization is required after a maximum of five visits to a dermatologist in a 12-month period for a dermatologic problem.
