Designated Provider Sample Clauses
Designated Provider. Tenant has informed Landlord that Tenant plans to utilize its own personnel in the installation, maintenance, and repair of the telecommunication cabling, wiring, and risers within the Premises. Tenant represents, warrants, and covenants that such personnel are and at all times shall be qualified to conduct such installation, maintenance, and repair of the telecommunication cabling, wiring, and risers. In the event that Landlord in its reasonable opinion determines that the installation, maintenance, and repair of the telecommunication cabling, wiring, and risers by Tenant's personnel creates a significant risk of adversely effecting the telecommunication cabling, wiring, and risers running through the portion of the Building not occupied by Tenant, Landlord may require and Tenant agrees to have the installation, maintenance, and repair of the telecommunication cabling, wiring, and risers done by an independent contractor approved by Landlord.
Designated Provider. Hub and Spoke pilot program sites were selected by the Department via a Grant Funding Opportunity Application (GFOA). Three authorized hub sites were selected to provide health home services and to contract for those services with authorized spoke sites. PIHPSs are expected to enter into MOUs with the DHS contracted hub and spoke pilot sites to coordinate services in the pilot program service areas.
Designated Provider. The term ‘‘designated provider’’ means a physician, clinical practice or clinical group practice, rural clinic, community health cen- ter, community mental health center, home health agency, or any other entity or provider (including pediatricians, gynecologists, and obstetricians) that is determined by the State and approved by the Secretary to be qualified to be a health home for eligible individuals with chronic conditions on the basis of docu- mentation evidencing that the physician, practice, or clinic—
(A) has the systems and infrastructure in place to provide health home services; and
(B) satisfies the qualification standards es- tablished by the Secretary under subsection (b).
Designated Provider. The Plan Provider chosen by the subscriber and/or members and designated by the Plan to be responsible for the general vision care of the subscriber/or member and their dependents, and who is receiving the consideration provided for hereunder for the vision care of such subscriber and/or member. All family members must choose the same Designated Provider.

Tenant agrees to have the installation, maintenance, and repair of the telecommunication cabling, wiring, and risers done by an independent contractor approved in writing in advance by Landlord, provided Landlord shall not unreasonably withhold its approval of such independent contractor.

Services may not be substituted or delegated to any other agent of the Contractor without express written approval of the County Health System Chief.

In the event that Landlord in its reasonable opinion determines that the installation, maintenance, and repair of the telecommunication cabling, wiring, and risers by Tenant's personnel creates a significant risk of adversely effecting the telecommunication cabling, wiring, and risers running through the portion of the Building not occupied by Tenant, Landlord may require and Tenant agrees to have the installation, maintenance, and repair of the telecommunication cabling, wiring, and risers done by an independent contractor approved by Landlord, which approval Landlord shall give or withhold within fifteen (15) business days following the receipt by Landlord of Tenant's request therefor.
