Common use of Diabetes Treatment Clause in Contracts

Coverage will be provided for Medically Necessary diabetes treatment and outpatient self-management training and educational services, including medical nutritional counseling at a CareFirst BlueChoice approved facility. Diabetic Supplies are covered under Section 12 herein. Diabetic equipment is covered under Section 11 herein.
