Disabled Child. A disabled dependent child is an eligible employee’s child or grandchild regardless of marital status, who was covered and then disabled prior to the limiting age or any other limiting term required for dependent coverage and who continues to be incapable of self-sustaining employment by reason of developmental disability, mental illness or disorder, or physical disability, and is chiefly dependent upon the employee for support and maintenance, provided proof of such incapacity and dependency must be furnished to the health carrier by the employee or enrollee within thirty one (31) days of the child’s attainment of the limiting age or any other limiting term required for dependent coverage. The disabled dependent is eligible to continue coverage as long as s/he continues to be disabled and dependent, unless coverage terminates under the contract.

The disabled 24 dependent is eligible to continue coverage as long as s/he continues to be disabled and 25 dependent, unless coverage terminates under the contract.
Disabled Child. A dependent child who is mentally or physically disabled and chiefly dependent upon the Enrollee for support and maintenance is eligible to continue coverage after the end of the calendar year in which the child turns age twenty-six (26). The employee must provide adequate documentation validating disability to Contractor prior to sixty (60) Calendar Days of termination of the dependent’s coverage. Coverage will be continuous. Contractor is responsible for determining whether the dependent meets total disability criteria.

Disabled Child. This Plan covers disabled children. A disabled child is a child who is and continues to be both (a) incapable of self-sustaining employment by reason of developmental disability, mental illness or disorder, or physical disability and (b) chiefly dependent upon the employee for support and maintenance, provided proof of such incapacity and dependency is furnished to the Plan Manager by the employee within 31 days of the child's attainment of the limiting age and subsequently as may be required by the Plan Manager, but not more frequently than annually after the two-year period following the child's attainment of the limiting age. Any notice regarding termination of coverage due to attainment of the limiting age will notify you of this right.
