Disclosure of Interested Parties (Form CIQ). Offeror must fully disclose the existence of any relationships as defined above in its response to this procurement packet. The Conflict of Interest Questionnaire (CIQ), attached hereto as Appendix “D”, must be filed with the Xxxxxxx County Clerk, located inside the Xxxxxxx County Courthouse, at 000 X. Xxxxxxx, Edinburg, TX 78539 no later than the seventh business day after the date the person becomes aware of facts that require the statement to be filed. Completion and submission of Form CIQ is the sole responsibility of the Offeror. Additionally, the Offeror must immediately notify Xxxxxxx County if the information provided in its response changes at any time.
