Common use of Disenrollment Clause in Contracts

Disenrollment. Participants may request to be disenrolled from the FIDA Plan or from the FIDA Demonstration altogether at any time for any reason, orally or in writing. A Disenrollment request may be made by the Participant to the Enrollment Broker or 1-800-MEDICARE.
