Division of Community and Family Health Services Payments. (A) When an Enrollee qualifies for special services offered through the Utah Department of Health’s Division of Community and Family Health Services (“DCFHS”), the Contractor agrees to reimburse DCFHS at the standard Medicaid rate for one outpatient team evaluation and one follow-up visit when the Enrollee becomes Eligible and selects the Contractor’s Dental Plan.
