Documenting Services Authorized by the MCO. The IDT staff shall give the member, as part of the MCP, a listing of the services and items that will be authorized by the MCO. The list shall include at a minimum: i. The name of each service or item to be furnished; ii. For each long-term care service, the units authorized; iii. The frequency and duration of each service including the start and stop date; and iv. For each service, the provider name.
