Documents Required from Subscriber. 3.1 The Subscriber must complete, sign and return to the Company an executed copy of this Subscription Agreement and if the subscriber is a resident of Canada an executed copy of the Accredited Investor Questionnaire attached to this Agreement as Exhibit “A” (the “Questionnaire”).
