ENTRY PER PERSON. There are two (2) ways to submit an entry (each, an “Entry”) to enter the Sweepstakes:

Proof of mailing does not constitute proof of delivery. Limit of one (1) Entry per person. Entries must be received during the Entry Period to be eligible. Any Entry received thereafter or not properly submitted will not be eligible to participate in the Sweepstakes. Any Entrant using multiple accounts to enter will be disqualified from the Sweepstakes and will not be eligible to win a Prize. OtterBox reserves the right to edit or request the Entrant edit their Entry (as deemed necessary by OtterBox in its sole discretion) and resubmit to comply with these Official Rules. To enter by mail, the Entrant must submit a properly completed Entry Form and mail it with first-class postage affixed and addressed to: “State Farm Teacher Appreciation Night Contest – Houston Rockets, Attn: Xxxxx Xxxxx, 0000 Xxxx Xx., Houston, TX 77002.” Entry Forms must be received during the Contest Period to be eligible. Any Entry Form received thereafter and/or not properly filled out will not be eligible to participate in the Contest. A person who timely submits a properly completed Entry Form in accordance with the procedures herein is considered an “Entrant.” By submitting a properly completed Entry Form, Entrants and Nominees are acknowledging that they have read, understand and agree to be bound by these Official Rules
