EXPRESS ASSUMPTION OF RISK. UNDERSIGNED hereby acknowledges and understands that the World Health Organization has declared COVID-19 a worldwide pandemic. COVID-19 is extremely contagious and spreads mainly from person-to-person contact. Based on currently available information and clinical expertise, older adults and people of any age who have serious underlying medical conditions may have a higher risk for severe illness from COVID-19. There have been recommended guidelines and preventative measures put in place to reduce the spread of COVID-19; however, it CANNOT GUARANTEE that the Minor will not become exposed to or infected with COVID-19, despite reasonable efforts to mitigate such dangers. Furthermore, the Activities could increase the Minor’s risk of contracting COVID-19. By signing this Agreement, UNDERSIGNED acknowledges and will inform the Minor of the extremely contagious nature of COVID-19 and voluntarily assumes the risk that the Minor may be exposed to or infected with COVID-19 from the Activities, and that such exposure or infection may involve the RISK OF SERIOUS INJURY, ILLNESS, PERMANENT DISABILITY AND/OR DEATH. UNDERSIGNED understands and will inform the Minor that the risk of becoming exposed to or infected with COVID-19 by the Minor’s participation in the Activities may result from the actions, omissions, or negligence of others and/or UNDERSIGNED, including, but not limited to, the RELEASEES (as defined below). UNDERSIGNED hereby expressly assumes all such risks and dangers whether presently known or unknown.

There have been recommended guidelines and preventative measures put in place to reduce the spread of COVID-19; however, it RELEASEES (as defined below) CANNOT GUARANTEE that the Minor will not become exposed to or infected with COVID-19, despite reasonable efforts to mitigate such dangers. Furthermore, the Activities could increase the Minor’s risk of contracting COVID-19. By signing this Agreement, UNDERSIGNED acknowledges and will inform the Minor of the extremely contagious nature of COVID-19 and voluntarily assumes the risk that the Minor may be exposed to or infected with COVID-19 from the Activities, and that such exposure or infection may involve the RISK OF SERIOUS INJURY, ILLNESS, PERMANENT DISABILITY AND/OR DEATH. UNDERSIGNED understands and will inform the Minor that the risk of becoming exposed to or infected with COVID-19 by the Minor’s participation in the Activities may result from the actions, omissions, or negligence of others and/or UNDERSIGNED, including, but not limited to, the RELEASEES (as defined below). UNDERSIGNED hereby expressly assumes all such risks and dangers whether presently known or unknown. UNDERSIGNED agrees that there is no liability for an injury or death of UNDERSIGNED participating in Activities if such injury or death results from the inherent risks of contracting COVID-19. UNDERSIGNED expressly assumes this risk by participating in such Activities.

By signing this Agreement, UNDERSIGNED acknowledges and will inform the Minor of the extremely contagious nature of COVID-19 and voluntarily assumes the risk that the Minor UNDERSIGNED may be exposed to or infected with COVID-19 from participation in the ActivitiesEvents, and that such exposure or infection may involve the RISK OF SERIOUS INJURY, ILLNESS, PERMANENT DISABILITY AND/OR DEATH. UNDERSIGNED understands and will inform the Minor that the risk of becoming exposed to or infected with COVID-19 by the MinorUNDERSIGNED’s participation in the Activities Events may result from the actions, omissions, or negligence of others and/or UNDERSIGNED, including, but not limited to, the RELEASEES (as defined below). UNDERSIGNED INDIVIDUAL understands and will inform the Minor that the risk of becoming exposed to or infected with COVID-19 by the MinorINDIVIDUAL’s participation in the Activities may result from the actions, omissions, or negligence of others and/or UNDERSIGNEDINDIVIDUAL, including, but not limited to, the RELEASEES (as defined below). Furthermore, if I become infected with COVID-19, I may expose others to the risk of infection. Activities could increase the Minormy and other’s risk of contracting COVID-19. By signing this Agreement, UNDERSIGNED acknowledges and will inform the Minor of I acknowledge the extremely contagious nature of COVID-19 and voluntarily assumes assume the risk that the Minor I may be exposed to or infected with COVID-19 from the Activities, and that such exposure or infection may involve the RISK OF SERIOUS INJURYrisk of COVID-19, ILLNESSserious injury, PERMANENT DISABILITY AND/OR DEATHillness, permanent disability and/or death to myself or others. UNDERSIGNED understands and will inform the Minor I understand that the risk of becoming exposed to or infected with COVID-19 by the Minor’s my participation in the Activities may result from the actions, omissions, or negligence of others and/or UNDERSIGNEDme, including, but not limited to, the RELEASEES (as defined below). UNDERSIGNED I expressly agree, covenant, and promise to accept and assume all responsibility and risks for injury, death, illness, or disease, or damage to myself, to others or to personal property of mine or others arising from my participation in the activities or event or my presence at a Rockingham County Parks and Recreation Activity or Event. I hereby expressly assumes assume all such risks and dangers whether presently known or unknown.
