Subject to section 12.05(3), the compliance fee for submissions that include only Operational Failures is as follows: (a) The fee for an employer with fewer than 25 employees is $500. (b) The fee for an employer with at least 25 and no more than 1,000 employees is $1,250. (c) The fee for an employer with more than 1,000 employees but less than 10,000 is $5,000. (d) The fee for an employer with 10,000 or more employees is $10,000.
