Filing of Claims. A claim for benefits shall be made by filing a written request with a person or committee designated by the Company (the “Plan Administrator”), which shall be delivered to the Plan Administrator and accompanied by such substantiation of the claim as the Plan Administrator considers necessary and reasonable for the type of claim being filed.
