Final Payment Verification. Contractor/Consultant shall submit a Contract Close-Out DBE Payment Certification (DBE-18) form completed by the contractor/consultant and each DBE along with its final request for payment, in addition to a final DBE Utilization Report (DBE-16) covering the entire project. Milwaukee County will not process the final request for payment without inclusion of these required forms.
