Final Program Report. Contractor shall electronically submit to Xxxxx Xxxxxx, Supervisor of the Partnership for the Arts & Humanities, a Final Program Report detailing program outcomes, challenges, successes, reach, etc. no later than Friday, September 30, 2020. The Final Program Report, a sample of which is included as Exhibit D, will be available one month prior to its due date. Failure to submit a timely Final Program Report may impact the processing and reimbursement of Cost Reports.

Final Program Report. A final report is due within 90 calendar days after the period of performance ends. The Final Report must be submitted online by recipients in the EHBs at xxxxx://xxxxxx.xxxx.xxx/webexternal/home.asp. The Final Report is designed to provide HRSA with information required to close out a grant after completion of project activities. Recipients are required to submit a final report at the end of their project. The Final Report includes the following sections: ▪ Project Objectives and Accomplishments - Description of major accomplishments on project objectives. ▪ Project Barriers and Resolutions - Description of barriers/problems that impeded project’s ability to implement the approved plan. ▪ Summary Information: • Project overview. • Project impact. • Prospects for continuing the project and/or replicating this project elsewhere. • Publications produced through this award activity. • Changes to the objectives from the initially approved award. Further information will be provided in the NOA.
Final Program Report. The UTM/DDA shall: (a) provide a Final Program Report which contains details about whether it has met all of the requirements in the Service Plan, as well as any other details requested by the Province; (b) ensure that the Final Program Report is signed on behalf of the UTM/DDA by the signing officers who have authority to bind the UTM/DDA in the format set out as Appendix 4 to Schedule “D”.
Final Program Report. The Recipient shall: (a) provide a statement about whether and how the Recipient completed the Program, including details requested by the Province; and (b) shall ensure that the final program report is signed on behalf of the Recipient by such number of signing officers as the Province may require. This report must include a description of all of the revenues and expenses of the Recipient, and must describe the extent to which the Recipient achieved its goals and objectives for the year as set out in its Annual Service Plan. If any of those goals or objectives were not achieved, the report must include a description of the reasons that they were not achieved, and the measures that the Recipient plans to take to achieve them. If the Recipient considers that any of those goals and objectives cannot be achieved, the report must include a description of the reasons that they cannot be achieved, and the measures that the Recipient plans to take to take as a result. Additionally, this final report must also document the outputs and outcomes of the services and activities identified in Schedule “A”.
Final Program Report. 1. The Collaboration shall submit or otherwise provide a Final Report making disclosure of all major developments by the Collaboration upon completion of the Agreement or within sixty (60) calendar days of termination of this Agreement. With the approval of the NAVSEA Program Manager, reprints of published articles may be attached to the Final report. The report shall be submitted electronically to the NAVSEA Program Manager and the NAVSEA Agreements Officer. One copy shall be submitted to the Defense Technical Information Center, Attn: DTIC- BCS, 0000 Xxxx X. Xxxxxxx Road, Suite 0944, Fort Belvoir, VA 22060-0944. 2. The Final Report shall be marked with a distribution statement to denote the extent of its availability for distribution, release, and disclosure without additional approvals or authorizations. The Final Report shall be marked on the front page in a conspicuous place with the following marking: “DISTRIBUTION STATEMENT B. Distribution authorized to U.S. Government agencies only to protect information not owned by the U.S. Government and protected by a contractor’s “limited rights” statement, or received with the understanding that it not be routinely transmitted outside the U.S. Government. Final Program Report. 15.1. Program Implementer will submit a final report that reviews the Program’s progress and accomplishments. At a minimum, the Final Report will cover: • Energy Savings – gross unverified annual and lifecycle MWh and therms achieved, by quarter relative to forecasts contained in Table1b. • Budget Expenditures (Incentive and non-Incentive), by quarter, relative to forecasts contained in Table1b. • Number of Customers, and location of Customers by geographic area. • Geographic distribution of Customers by year, in map format (using ArcGIS or similar software). Customer participation shall be by urban versus rural areas and by utility service territories for both gas and electric utilities. • Number and type of Measures, and total Program savings by Measure. • Performance Relative to KPIs contained in Table 3. • Program coordination and leveraging. • Marketing and Communications: Analyze and describe which program marketing and communications strategies were most cost-effective, and which were less cost-effective. • Successes – description of ways in which the Program was successful. • Continuous Improvement – description of how the Program could be improved to either increase Program penetration or Customer satisfaction, or reduce costs. • Description of how Program was improved each year from the beginning of the Program. • Customer and Trade Ally Satisfaction - Describe and analyze Customer and Trade Ally satisfaction and how they could be improved. • [Add Program-Specific Requirements] Task 15: Final Program Report 15.1.
