For Group Services. 3.7.4.1. When a group service is rendered, a list of participants is required to be documented and maintained by the provider. 3.7.4.2. Every participant shall have a progress note in their clinical record that documents the service encounter and their attendance in the group, and includes the information listed in 15.3 above. 3.7.4.3. The progress note for the group service encounter shall also include a brief description of the members’ response to the service.
