For Younger Youth Participants. (a) Contractor shall distribute the Participant Evaluation forms to Project Staff. (b) Contractor shall require Project Staff to complete two (2) evaluations for each Participant: (i) The first evaluation shall be completed at the end of the third week of the Program Period; and (ii) The second evaluation shall be completed during the final week of the Program Period. (c) Contractor shall collect completed evaluation forms for each Participant from Project Staff.
