Formulary Management. Pharmacy shall provide formulary management.
Formulary Management. Navitus shall provide a recommended drug formulary to Client. Client agrees to implement, administer, and cooperate with Navitus and to facilitate Eligible Persons’ utilization of the Formulary.
Formulary Management. Pharmacy Benefit Manager will provide a recommended Formulary to State, subject to modification by State. Pharmacy Benefit Manager may modify the Formulary from time to time as a result of decisions of its Pharmacy and Therapeutics Committee and new therapeutic agents that become available, subject to modification by State. Pharmacy Benefit Manager will notify State in advance of modifications to the Formulary. State will implement, administer, and cooperate with Pharmacy Benefit Manager and facilitate Members’ utilization of the Formulary. Pharmacy Benefit Manager will maintain the Plan’s Formulary on-line via its website and cooperate with State to ensure that it is in a format that facilitates State’s ability to establish links between MMB’s website and the internet version of that Formulary.
Formulary Management. 2.5.1.1 Blue Cross NC will provide Formulary clinical services in accordance with NCQA standards and all applicable state and federal laws. 2.5.1.2 Blue Cross NC will provide Formulary management services and will update the Formulary in a timely manner. 2.5.1.3 Blue Cross NC will coordinate Pharmacy & Therapeutics (P&T) Committee meetings at least quarterly and include developing agenda, therapeutic class reviews, drug monographs and quarterly updates of Formulary publications available on Blue Cross NC’s website.

Formulary Management. PBM will provide an electronic Formulary to Participating Group. If Participating Group is using a custom Formulary the Participating Group shall pay PBM a fee as listed in Exhibit C to manage the Formulary. PBM shall build and support a compliant and searchable Formulary web file for all lines of business. Participating Group agrees that any Participating Group-imposed changes to the Formulary may increase, reduce, or eliminate the amount of Rebates to Participating Group. PBM agrees to provide thirty (30) day notice to Participating Group and Client of any changes to the Formulary used for the pricing models listed on Exhibit C.
