Grant Agreement Documents. This Grant Agreement, including the documents annexed hereto as Attachments A-G, constitute the complete agreement of the parties. The Attachments are as follows: Attachment A – Scope of Work Attachment B – Budget Attachment C – Source of Funds Attachment D – Method of Payment Attachment E – Reporting Requirements Attachment F – Additional Flexible Facilities Program Rules Attachment G – Federal Compliance Requirements for Capital Projects Funds
Grant Agreement Documents. The documents listed below are incorporated by reference and made a part of the Grant Agreement for all purposes. In the event of a conflict, ambiguity, or inconsistency between the terms and conditions set forth in the documents that comprise the Grant Agreement, including all amendments hereto, the controlling document shall be this Signature Document, then the remaining documents in the following list in the order stated:
1. ATTACHMENT A: CONTRACT AFFIRMATIONS (VERSION 2.3);
2. ATTACHMENT B: UNIFORM TERMS AND CONDITIONS – GRANT (VERSION 3.4); and
Grant Agreement Documents. This Grant Agreement consists of this document and its attachments; (a) Terms and Conditions (Attachment A), and (b) Grantee’s Work Plan (Attachment B). This Grant Agreement comprises the entire agreement between the Parties and supersedes any and all previous and contemporaneous agreements and representations, whether oral or written. The Parties may amend the Grant Agreement as provided in Paragraph 8.
Grant Agreement Documents. The following documents are incorporated by reference and made a part of the Grant Agreement for all purposes. In the event of a conflict, ambiguity, or inconsistency between the terms and conditions set forth in the documents that comprise the Grant Agreement, the following documents shall control in the specified order of precedence:
1. This five (5) page Signature Document;
2. ATTACHMENT A: CONTRACT AFFIRMATIONS (VERSION 2.3);
3. ATTACHMENT B: UNIFORM TERMS AND CONDITIONS – GRANT (VERSION 3.2);
4. Attachment C: Additional Provisions;
5. Attachment D: System Agency Open Enrollment No. HHS0014031; and
6. Attachment E: ▇▇▇▇▇▇▇’s Response to System Agency Open Enrollment No. HHS0014031.

Grant Agreement Documents. The following documents shall be returned to the Department via email to ▇▇▇▇@▇▇▇▇.▇▇▇▇.▇▇▇. Emails must be sent with the following in the subject line: “County Name_ Administrative Agency Name_ Service Coordination Grant_July 2018”. Documents attached to the email must be clearly labeled.
1) A signed Agreement.
2) A completed Administrative Agent Fiscal Contact Form (Addendum B).
3) A completed Federal Funding Accountability and Transparency Act (FFATA) Form (Addendum C).
4) If combining allocation via a multi-county agreement, a copy of the agreement between the collaborating counties. The following documents shall be submitted to the Department via attachment in EIGS when submitting the budget.
1) If Grant funds will be used for indirect costs, a copy of the Grantee’s federally approved indirect rate or a modified total direct cost worksheet (Exhibit B).
2) Program Narrative/Budget Summary. Directions for completing this document are included in the Program Narrative/Budget Summary Instructions (Exhibit C).

Unless expressly stated otherwise in this Grant Agreement, in the event of conflict, ambiguity or inconsistency between or among any documents, all System Agency documents take precedence over ▇▇▇▇▇▇▇’s documents and the Data Use Agreement takes precedence over all other Grant Agreement documents. o Attachment A: Statement of Work; o Attachment B: HHS Uniform Terms and Conditions – Grant Version 3.5 (September 2024); o Attachment C: Supplemental conditions; o Attachment D: Federal Assurances; o Attachment E: Certification Regarding Lobbying; o Attachment F: FFATA Certification Form; and o Attachment G: Data Use Agreement (October 2019).
