Indiana Residents. These securities have not been registered under Section 3 of the Indiana Securities Act and therefore, cannot be resold or transferred unless they are so registered or unless an exemption from registration is available.
Indiana Residents. Proof of payment for this Service Contract constitutes proof of payment to the insurer identified in the Coverage and Term section, reverse, for reimbursement insurance coverage specified in that section. This Service Contract is not insurance and is not subject to Indiana insurance law.
Indiana Residents. The undersigned hereby represents that he is acquiring the Securities for his own investment and that he is aware that transferability and resale of the Securities is restricted by state and federal law.
