Individual Profile Questionnaire. Applies to investigative Vendor Staff only. Vendor shall provide Exhibit C, Individual Profile & Questionnaire, within thirty (30) calendar days of the execution of this Agreement or as otherwise directed by Citizens. An Individual Profile Questionnaire should contain detailed information regarding relevant work history with dates, certifications and related training to work as outlined in this Agreement. The Individual Profile Questionnaire shall substantiate the individual’s investigative experience and match information the Vendor inputs for each Vendor Staff within CAIS.
Individual Profile Questionnaire. (Applies to Investigative Staff only) Vendor shall provide a Individual Profile Questionnaire, attached hereto as Exhibit D, within 30 calendar days of the Contract effective date or as otherwise directed by Citizens, and continually as staff is added to work being conducted for Citizens. A Individual Profile Questionnaire should contain detailed information regarding relevant work history with dates, certifications and related training, and client references related to work as outlined in this contract. The Individual Profile Questionnaire should substantiate designee’s investigative experience and match information the Vendor inputs for each designee in the Qualifications Tab within CAIS.
