INSURED PERSON'S RESPONSIBILITIES. It is the Insured Person's RESPONSIBILITY to initiate the external appeal process. The Insured Person may initiate the external appeal process by filing a completed application with the New York State Insurance Department. The Insured Person may appoint a representative to assist with the Insured Person's external appeal request, however, the Insurance Department may contact the Insured Person and request that the Insured Person confirm in writing that he or she has appointed such representative.
